Thoughtful medical care, wherever you call home.
Medica Home Calls offers relationship-centered care for people with chronic or serious illness—in houses, senior communities, and other supportive settings. We meet you where you are, at a pace that respects your story.
Who we are
Medica Home Calls serves as a bridge between hospital care, specialists, and the everyday realities of living with serious illness at home. Our visits are intentionally unhurried, with room for questions, stories, and difficult decisions.
Our clinical focus
We focus on adults living with chronic and advanced conditions, particularly those who feel stretched between multiple doctors, medications, and care settings.
- Heart and lung disease
- Dementia and cognitive change
- Depression and anxiety
- Diabetes and metabolic conditions
- Frailty, falls, and complex aging
How we show up
Visits can be at the kitchen table, bedside, or over secure video. We coordinate closely with your existing primary care, specialists, and home health teams whenever possible.
Our aim is to add clarity, not replace relationships that are already working well for you.

Meet the Founder
Patrick Blanchard, M.D.
After more than three decades practicing family medicine and home-based supportive care, Dr. Patrick Blanchard founded Medica Home Calls to bring medicine back to its most human roots... presence, listening, and continuity of care.
Through years of work in traditional clinics, hospice care, and leading his own home-visit practice (ValiseMD), Dr. Blanchard repeatedly saw patients struggle with rushed appointments, fragmented systems, and the difficulty of accessing care when illness made travel impossible.
Medica Home Calls was created to offer something better — unhurried visits in the comfort of home, close coordination with existing teams, and care centered on a patient's goals rather than schedules.
Board-certified in Family Medicine and fluent in English and Spanish, Dr. Blanchard remains deeply involved in every patient visit, believing that meaningful medical care begins with listening.
- Board-certified, American Board of Family Medicine
- 30+ years clinical experience
- Hospice & palliative care background

How we hope care feels
Our work is guided by a few simple commitments: to slow down, to listen closely, and to respect the limits of medicine while still offering real help.
Clarity
We name uncertainty and trade-offs honestly so that you and your family can make decisions without pressure or jargon.
Continuity
We aim to be a steady presence across hospitalizations, rehab stays, and moves between settings.
Compassion
We recognize that serious illness reshapes identities, families, and futures. Our visits make space for that reality.
